Give timely notice for the end of the property rental contract (the exact period depends on the contact terms).

Cancel all utilities and contracts: internet, mobile phone, gas, electricity, TV, and others.

Deregister from local Town Hall (in many European countries individuals are required to obtain a Certificate of Discharge from the Population Register, or similar)

Hand over the apartment following a strict protocol. Be aware that if this protocol is not followed, there might be penalties to pay or your transferee might not receive their entire rental deposit back.

Handle medical insurance and/or pension contributions.
